Hi Mark, 
yes you are right..it seems that by copying and paste something went wrong. The command should be the following:
xtivreg2 lnct3 $rhs (q12c q21c q16c q17c= $QIs_prev $iv1), fe cluster(cpa)  orthog($QIs_prev $iv1) endogtest(q12c q21c q16c q17c) small ffirst

I am using Stata 12.1, and just tried to install again the routing of xtivreg2 using SSC and replacing the previous routine.

There are syntax errors in what you say you ran.

xtivreg2 lnct3 $rhs (q12c q21c q16c q17c=3D$QIs_prev $iv1), fe cluster(cpa)=  orthog($QIs_prev $iv1) endogtest(q12c q21c q16c q17c) small ffirst

has an "=" after the cluster option, and "3D$QIs_prev" will give you an illegal variable name, I think.  So this is probably not exactly what you did.

In any case, can you (as per the Statalist FAQ) tell us basic things like which version of Stata you are using, which version of xtivreg2, etc?
