I should have remembered: -gllamm-, available from SSC, has a cluster option for longitudinal and multilevel models with logit response. The manual can be downloaded from http://www.bepress.com/ ucbbiostat/paper160/ Stas Kolenikov has a demo at http:// www.unc.edu/~skolenik/stata/gllamm-demo.html
