If you have Stata 9 or higher, I suggest you get -mim- (ssc install mim). mim seems to be a superior alternative to micombine, and given that Royston is an author or co-author of both I am not sure why both continue to exist or why they have slightly different requirements for the setup of the data file. Anyway, an example from the help file is

mim: svy: logistic heartatk age weight height
