Stata Tech Support found that I had a hidden worksheet that had fewer columns, and the hidden worksheet was the one that was being imported by default. I'm not sure how this happened (since I didn't create it) and I'm not sure how to fix it in Excel since the "Unhide" button is grayed out. However, all's well that ends well since I can choose the correct sheet on the command line or in the File->Import Excel dialog.

I have no idea what software produced the Excel files, as we received them from somewhere else. They had an .xlsx extension, so I assume MS Excel 2010 or 2013, but I can't say for sure.
I used MS Excel 2010 to save the file under a new name. I also tried saving as .xls.
