Be careful in attributing all the difference to e.g. improving
efficiency of health care. Change in the prevalence of a disease may
occur e.g. because of the change in the population structure (e.g. if
children are more susceptible to the virus, and the population has
changed so that there is fewer children) and other factors (not
terribly important if your rates are really for two subsequent years).

Also you need to specify what do you mean by "significant". At least
statistically significant? economically significant?
