Tony, I think the source is Peter Nemenyi's unpublished Ph.D. thesis (Princeton, 1963). Erich Lehmann's nonparametric book mentions it. John Tukey's manuscript "The Problem of Multiple Comparisons" (1953?) was eventually published in the 1980s in Volume 8 of The Collected Works of John W. Tukey.
