It would help to know more about the "pile-up of responses at zero."
Do the frequencies of positive scores tend to decrease, with the
largest frequency at 1?  Or do the positive scores have a mode at some
value greater than 1?  Those are two kinds of zero-inflated mixtures.
You may need a mixture or two-part model.
