Thank you Joseph for your suggestions. They put me on the right track.

It appears that if patients at clinics are either all treated or all untreated that I was wrong in my earlier comment that all procedures fail: Although xtlogit with fixed effects does not work, xtlogit with random effects seems to work fine, (and gives identical results as xtmelogit).

Thank you also for hinting me on clchi2.
