The -charlson- program requires ICD9 or ICD10 coding of the
co-morbidities.  So if you know the disease state, but it is not
ICD-coded, then your only option would be to code them.

Also, I believe there is a bug in the -charlson- program.  I'll track
that down later today when I get into work and post the correction.
