st: new program rct_minim

Thanks to Kit Baum a new program -rct_minim- is now available on the SSC
server.

-rct_minim- provides treatment assignments to subjects in a controlled
clinical trial such that imbalance in prognostic factors is minimized.
It is based on Pocock and Simons' paper (Biometrics 1975; 31:103-115; full
ref in the accompanying help file).

Requires Stata v10 and Paul Millar's -matsort- command (also available from
the SSC server).
