1. You can check out this new World Bank on Analyzing
Health Equity Using Household Survey Data by Owen
O'Donnell, Eddy van Doorslaer, Adam Wagstaff, and
Magnus Lindelow. I think you will be interested in
chapter 3 (Health outcome#1:Child survival)
specifically. They describe how to compute mortality
rate from household survey data using Stata.
