Can't advise you on your latter question, but if you have very few 1s
and lots of 0s in your response variable, you should try fitting your
model with -relogit-, a user-written pacakage by Gary King and Langche
Zeng which is downloadable by SSC.

(Note, however, that their latest version of this routine is written
for R but _not_ for Stata.)
