st: Nested Logit: Individual level parameters and degenerate branches?

Can someone explain to me why individual-level parameters (that do not
vary with alternatives) cannot be put in the lower level of the nested
logit?

Also, how do you get Stata to handle degenerate branches (i.e.
branches which do not have multiple alternatives attached to them)
appropriately?
