st: zanthro and the new 2006 WHO growth reference

I'm one of the authors of the zanthro function.  I'm planning the following 
updates:
1. Add thinness cut-offs to zbmicat
2. Allow for correction of prematurity for the British growth charts.
3. Include the WHO growth reference.

I'll probably do this in 2 stages - publish steps 1 and 2 and later add the 
WHO growth reference. I'm very busy at the moment and won't get a chance to 
do this until next year.  Stay tuned for these updates in 2008.
