For an example, see the book Multilevel and Longitudinal Modeling
Using Stata, pages 169-175. The book is available from the Stata
bookstore at http://www.stata.com/bookstore/mlmus.html. The idea is to
use the thresh() option to relax the proportional-odds assumption.
