Dear Steve and Richard,

many thanks for your comments. They have been very helpful. 
I really believe that the item in the Richard’s wish-list is worth noting. I will raise this point in the next Stata Users meeting. I didn’t know the Kronmal (1993) paper. Very interesting. I fully agree with his arguments. Thanks Steve for pointing that out.
