Thanks, Mark. I copied the cluster2.ado but had some issue implementing it. While I had version 10, I have updated it all. But when I use ivreg2, I still receive the following message:

this is version 10.0 of Stata; it cannot run version 10.1 programs
     A free update to version 10.1 is available; type update and follow the instructions, or visit http://www.stata.com/support/updates.  If you are not connected to the
     Internet, you can contact StataCorp to obtain the latest version.
(error occurred while loading ivreg2.ado)

It might be easier if you use -ivreg2-.  The latest version does 2-way clustering, for OLS as well as IV/GMM.
