This does not seem a complete question.
What you mean by scoring?
Do you have data for week 24 or not?
If you do not, how do you expect to predict week 24 from other weeks?

I am trying to find a way to score number of pills intake per patient
(cohort study) at week24:
npill1 and npill2 are variables with values 0 or 1 (i.e. took pills or
not) week number are 4,8,12,16
studyid week npill1 npill2
1001 4 1 1
1001 8 0 1
1001 12 0 0
etc..
