How does one format the data and/or what options should be used with
the stset command to allow time-varying covariates?  Unfortunately, I
don't have access to the ST manual, and the online help is somewhat
unclear on this issue.  The online help talks about "multiple record"
per unit data, but it's unclear whether this refers to multiple events
or about yearly data points for a single event.

(There is an FAQ entry on Stata 6 about this, but I'm wondering
whether Stata 9 has made these easier to implement.)
