I made a small but critical mistake: 'transplantdate' should be set to
some value larger than the date of the latest failure or censoring, if
the respondent did not receive transplantation until the end of the
study. If 'transplantdate' is set to missing (as above) these cases will
falsely be assigned to the group which received transplantation.
