I am not sure I understand your distinction between cause-specific and
competing risk hazard, and perhaps you could eleborate.

Meanwhile my own personal take on (independent) competing risks models is
set out in chapter 8 of the Survival Analysis manuscript downloadable at
http://www.iser.essex.ac.uk/teaching/stephenj/ec968/pdfs/ec968lnotesv3.pdf
It covers both continuous and discrete time survival time data cases.
The chapter includes references to other literature.
